Simfreaks has a huge pirate theme made up of both skins and objects and it's offered complete with a story to boot!! (There's also a very neat lot to be downloaded)
http://www.simfreaks.com/home.shtml

Vita Sims has Pirates of the Caribbean skins
http://www.vitasims.com/movies.htm (Site seems abandonded June 2008. I got the pirate skins at this link: http://www.vitasims2.narod.ru/ Edit by bruja)

Cómhair Sims has Pirates of the Caribbean skins.
http://comhair.planetthesims.gamespy.com/frames/skins_potc.htm
(While the objects section hasn't a "pirates" theme there's a ton of great items to be found that would fit very well.)

Eisbaer's Kreations has added a pirate's wife skin
http://de.groups.yahoo.com/group/eisbaers_creations/

The Coconut Hut is the first place I think of for pirate goodies (as well non-tiki tropical downloads. good stuff)
Site Closed

Dincer hepguler has some great cannons among his other goodies.
http://moonsims.asi.org/dh-sims-site/

Poetica's Themes' "Fishing & Boating Mini Theme " isn't especially piratey but that "boatbuilding" piece is just too cool to not make it onto the list.
http://games.groups.yahoo.com/group/poeticaplace_themes/
Poetica's Place also has sand and water rugs for slopes (in Outdoors folder)
http://games.groups.yahoo.com/group/poeticaplace/files/

Aponee Sims Yahoo group has some VERY piratey "Old Market Deco" (misc old market.zip) which is made up of barrels of foodstuffs, a pile of cannon balls and a barrel of gunpowder.
https://groups.yahoo.com/neo/groups/aponeesimz/files/Sets/Page%202/Misc%20Old%20Market%20Deco/
Pirate hats and boots
https://groups.yahoo.com/neo/groups/aponee_simz1/files/Sets/Page%2011/Pirate%20Hats%20and%20Boots/

Bag of Tricks' medieval viking objects work amazingly well in a pirate theme as does a lot of the Vintage General Store clutter and the Medieval Faire items.
http://games.groups.yahoo.com/group/bag_of_tricks/

The Blarney Stone Pirates by Dr. Pixel (skins)
Maxx's Female Buccaneers (skins)
and The Pirate's Cove (made up of Lots and objects created by Aunt B, Bag of Tricks and The Secret Society of Woobsha) can all be found at

warning! TA sims items are old and were made without a magic cookie and may need to be recloned or fixed to work.

Eph's Den of Many Sims has some male pirate skins - go to Male Fashion and have a browse around the casual skins. There are heaps of other male and female skins that could fit in the theme too.
http://sims.ephralon.de/index.php

And Comhair's Royal Navy (Hornblower) skins in the Regency period could make for interesting interactions with our pirates... wink

They may be a bit flashy for serious pirate themes, but Shannanisms' swashbuckling "Scurvy Dames" are unquestionably pirates, complete with buyables, saber, and a fabulous hat. They can be reached through the "Characters" link on the site's upper menu bar:

There are some great pirate skins in the Costume Shop at the Sim-Pi Event Land Fall 2009 site!

http://simp1.web.fc2.com/

After the event they will not be available for some time...
