If you already have houses and families on some of the lots, will this mess them up? Or can you add these neighborhood screens after you have created your families.

No, it doesn't matter. The lots are independent from the neighbourhood screen. The screen is like a wallpaper - it's only there for looks, it doesn't actually do anything in the game.

Sometimes it will happen that a lot will overlap features on the new screen, like roads or water.
If that bothers you, you could use the map_edit on cheat to alter a few squares of that lot to make it fit better. It isn't necessary to play, but some people (like me) like the 'hood view to be 'neat'. :)
